Steps to make an Appointment

  • Three options:

    • Complete Request for Information or Appointment link below

  • Complete Client Information,  Waiver and  Veterinary Approval  Forms:

    • Complete forms Online

    • Print forms from the Forms tab, OR Request forms to be emailed to you​- bring to first appointment

  • Warm Paws reviews information including Veterinary notes and recommendations

  • Call Warm Paws, or Client will receive a call to schedule first appointment

  • First appointment: Initial Evaluation, Review of Goals and Introduce the dog to the Pool

  • Plan subsequent sessions, as appropriate with owner

  • Referring Veterinarian and client receive status and progress notes​

Complete Client Request Form - Warm Paws will contact you, email forms if  needed, and schedule an appointment, as requested.
