Steps to Make an Appointment

  • Complete Request for Information or Appointment link below

• Step 2: Complete Required Forms (complete online, print OR request forms to be emailed to you)

  • ​Forms needed

    • ​Client Information

    • Waiver

    • Veterinary Approval  needed if under veterinary care for post injury or surgery

• Step 3: Warm Paws reviews information including Veterinary notes and recommendations

• Step 4: Call Warm Paws, or Client will receive a call to schedule first appointment

• Step 5: First appointment:

  • Initial Evaluation

  •  Review of Goals

  • Introduce the dog to the Pool

• Step 6: Plan subsequent sessions, as appropriate with owner

  • Referring Veterinarian and client receive status and progress notes​
